After-hours Contact Information
 | For after-hours questions, urgent problems and
emergencies, you are asked to contact our nurse triage service at 1
888 267 3675. Please provide this registered nurse (RN) from
UNC-Chapel Hill with our practice/doctor name, details of your
problem or concern. Please follow their recommendations, as
our doctors have approved the protocols they will guide you
through. This is the same advise that our providers would give
to you. This is a very expensive, but extremely safe and
effective service that Triad Medicine and Pediatric Associates, PLLC
pays
for from our own funds. We ask that you follow their instructions
and do not insist on speaking with a one of our providers unless the
nurse feels it is necessary. If the RN feels it is necessary,
a provider form Triad Medicine and Pediatric Associates, PLLC or our
covering practice will contact you directly. |

| BAD WEATHER POLICY: When Rockingham County
Schools have changes in their schedule due to weather, our office
will follow the following changes:
 | For school closing: Our office will open
at 11:00 am |
| For school having a 2-hour delay: Our
office will open 1 hour late, at 9:00 am |
| Please call our office for clarification.
Our staff will be trying to get to the office in accordance with
the above changes. |

Office Policies
| Our office follows strict Federal Regulations regarding privacy of
your medical information, better known as HIPAA. You will be
required to review our HIPAA policy and sign a statement agreeing to
these federal requirements. |
| Payment for services in our office are DUE AT THE TIME OF
SERVICE. |
| You will be required to provide full co-payments PRIOR to being
seen by a medical provider. Your insurance company REQUIRES
that we collect all co-pays at the time of service. |
| We will, at our discretion, file your insurance paper work for you
at the time of your visit. |
| You will be charged for missed appointments. Please remember when
an appointment is missed, it prevents us from seeing someone else
that may need urgent care in our office. Someday that someone
could be you or your child. |
